Acupuncture treatments are by appointment only.
  1. For an appointment that best suits your schedule, it is recommended to schedule 1-2 weeks in advance.

Insurance

Cancellation Policy
Many insurance companies now cover acupuncture. Check directly with your insurance company for specific details of your policy. I provide an itemized medical bill that can be submitted for reimbursement. Payment may be done with cash, check or credit card and is due at time of service
Please give 24 hours notice for cancellation of your appointment. With exception of emergencies, you will be billed for the missed appointment.

Your first visit begins with completing my New Client Forms.
  1. DOWNLOAD AND SAVE the PDF forms to your computer.
  2. COMPLETE AND PRINT the fill-able PDF forms.
  3. BRING THEM to your first visit.

2. Medical Records
Bring pertinent copies of lab tests and medical records to your first office visit.

3. Meal or Snack
Have a small meal/snack 1-2 hours prior to your session. Hydrate well.
